Stupid is as stupid does (no, this comment is not directed at you).
Shame his schizophrenia derailed his mental functionality. ECT is nasty and in my opinion borderline barbaric -- but back then they simply did not have the medications we have now. And it does help some people.

The first stanza sounds like someone satisfied with their life to date yet still seeking the reassurance of others that his life was actually meaningful.
The second stanza sounds like someone who's considering offing himself and is pretty sure no one would even notice.
Liked the song, found the video less than stellar
